Waverly facility skipped protocolÂ
The Nebraska Department of Human Health Services has completed its investigation into how a woman was mistakenly pronounced dead at a Waverly nursing home last month.
According to the report, a registered nurse working at The Mulberry at Waverly failed to follow the facility's own policy before declaring the woman dead the morning of June 3, which resulted in her being transported from Waverly to a Lincoln funeral home while she was still alive.
Investigators interviewed facility staff and others after the incident involving the 74-year-old woman, who was on hospice care because of a terminal illness.

Teen accused of train derailment
Investigators now believe a train derailment on the edge of Bennet in April was caused intentionally by a teenager who then recorded it and posted it online.
BNSF Railway Police have referred the 17-year-old boy to the Lancaster County Attorney's Office for prosecution for felony criminal mischief, saying the derailment caused an estimated $350,000 in damage.
In a search warrant filed this week, a BNSF police investigator said that shortly after 6 p.m. April 21 a fully loaded eastbound coal train derailed at a railroad crossing at Monroe Street and Nebraska Highway 43.
Fleet Farm backs out of development
And, a major commercial development in northwest Lincoln is regrouping after Fleet Farm, the big box retailer the developer had been negotiating with, backed out.
The Falcon Corner project, on the east side of Northwest 48th Street, included five commercial lots with a large retailer, which developers hoped would be Fleet Farm, and an outdoor storage yard on 17 acres, with the possibility of a gas station, car wash and two restaurants on the smaller commercial lots.
But Mark Hunzeker, who represents a limited liability company called FFF that owns the land northeast of Northwest 48th Street and Interstate 80, said they’re continuing to move forward and have graded the site
