Luckie's Lounge & Grill, 1101 W. Bond Circle. Sanitizer not measured at dish machine, repeat. Beef, turkey, cheese observed on rail at incorrect temp, repeat, check temps more frequently, corrected. Mop sink not draining, repeat.

Panera Bread, 201 N. 12th St. Employee(s) working without having current food handler permit. Time stamps not available for salad mixing bowls (in use), corrected.

Village Inn, 111 S. 29th St. Suspended as of Friday, Nov. 3. Cockroaches noted in all stages on sticky traps, many still alive. Phone call to supervisor resulted in immediate suspension.
